Job Responsibility:
Duties and responsibilities: Service delivery and development Be responsible for the oversight and management of the Memory Navigation Service for Bedfordshire. (Home - Memory in Bedfordshire (memoryinbeds.org.uk) Manage a team that meet the needs of adult carers that care for someone with memory loss, dementia or a mental health condition. Contribute to delivery of early response service; working with early response team to ensure registration and triage service is effective and efficient for dementia and mental health carers Work with colleagues in workshop, events and groups team to ensure activities meet the needs of adult carers and is evidenced by data and feedback. Identify areas of unmet need and work with colleagues to develop ideas and projects so Carers in Bedfordshire can secure funding to address them Maintain your own caseload of carers and the demand of the service Work with partners and stakeholders such as Tibbs Dementia, Admiral Nurses and Commissioners Act as adult safeguarding lead and ensure adult carer support in the locality operates within local Safeguarding Board practice, liaising with Child Safeguarding as appropriate Monitor and evaluate activities to ensure contract outcomes, carers wheel and KPIs are achieved, documented and the service is evaluated. Act and respond appropriately to customer satisfaction and quality outcomes. Prepare quarterly reports and data for the Memory Navigation Service contract and other funders as required Be the driving force and contribute to the service and organisational innovations and developments Be proactive and drive for change on new projects that meet strategic priorities and objective as appropriate that meet gaps based on evidence Ensure the service and external funding application are delivered within budgetary constraints working with Adult Services Lead and the Finance Manager Build budgets for the development of any funding applicationsTeam management and development: Manage a team that support dementia carers, mental health carers and other contracts as required Support team to collect data in a timely manner Effectively manage and motivate team members and volunteers in line with organisational policies, including induction, supervision, appraisal and training to deliver challenging targets Support team members to manage their work, including case supervision, case meetings, KPIs Ensure quality assurance processes are upheld and responded to Support wider team members and advocate on behalf of carers where required, for example with Local Authority social work teams, health professionals, other agencies, etc.Other responsibilities: Represent Carers in Bedfordshire and act as a local ambassador for carers Be aware of and develop relationships with statutory and third sector services involved with carers, including attending and reporting back on Partnership Boards and other multi-agency meetings. Support outreach and awareness raising work, including Carers Week Ensure adherence to good practice, adhering to, reviewing and/or developing policies and procedures as appropriate Keep up to date with changes in legislation and local services which may affect the lives of carers and/or impact our service Ensure personal information is collected, stored, and managed appropriately ensuring confidentiality of information as appropriate, including compliance with Data Protection legislation and guidelines Take responsibility for assigned areas of organisational risk Ensure central information systems are up-to-date including shared and personal calendars, service documentation Monitor and maintain a safe and secure working environment at all times. Participate in training and other organisational briefings and activities and maintain an up to date knowledge of carers needs and Carers in Bedfordshires services Attend regular supervision sessions and staff/team meetings Undertake such personal training as may be required to keep up to date and fulfil professional requirements Have a collaborative and flexible approach to work Some evening and weekend working is expected as part of your normal designated hours, and occasional nights away from home may be required Additional Duties: This job description is not exhaustive and services to highlight the main requirements of the post holder.
