Job Responsibility:

The Hospital to Home Service is a highly motivated multidisciplinary team which works across Dorset to support people leaving mental health wards and returning to their homes. The role of the Occupational Therapist will be to undertake OT-informed assessment and interventions to enable people to leave hospital is a safe and timely manner. The Occupational Therapist will provide support to the other members of the team (including Peer workers, Occupational Therapy Assistants, Support Workers and Housing Officers) as well as deputising for the service lead when required. To be responsible for providing and overseeing the delivery of evidence-based occupational therapy interventions to patients under the Hospital to Home Service. To provide clinical leadership to members of the Hospital to Home service. To undertake OT assessments, multidisciplinary and multi-agency care planning, case management and co-ordination, review, liaison, record keeping, clinical risk assessment and discharge planning of patients under the Hospital to Home Service. To plan and implement client centred individual and group interventions using graded activities to achieve therapeutic goals. 1To visit patients in their own homes to monitor, evaluate and modify treatment in order to reassess progress and ensure effectiveness of intervention using evidence based/standardised assessments and outcome measures. To work innovatively and creatively to engage patients and their carers in the treatment process. To work in partnership with patients and carers, dealing sensitively with their concerns, educating about illness and treatment and supporting them to access community resources. To assess and provide adaptive equipment necessary to promote the patients independence. To exercise a reasonable degree of professional autonomy and make judgements and decisions in order to satisfy the expectations and demands of the job.
