Job Responsibility:

To improve health and deliver excellent care. Put patients first, Work together, Value and respect each other, Learn and innovate, Communicate openly and honestly. To work independently under the direction of a qualified clinician to provide an Integrated service for Long Covid and Community Complex C onditions patients within BCUHB. To implement and deliver an exercise programme for the Community Complex Conditions Service. To work under direction of a physiotherapist/OT/clinician but in an unsupervised capacity to undertake assessments, treatment planning an intervention with individual clients or in groups, following agreed protocols and care pathways. To assess patients with simple physical conditions who require walking aids in keeping with departmental guidelines /standards. To deliver routine physiotherapeutic treatment programmes independently and progress treatments within guidelines and protocols. To be accountable for these tasks. To carry out activities relating to health promotion for Long Covid/CCC with a range of health and social care, professionals as well as leisure and voluntary organisations. To participate in training of self and others to update and assist in service changes within designated area. To be flexible and be prepared to work in other areas depending on service needs. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
