Job Responsibility:

The post holder will hold an MSc in Advanced Clinical Practice or Credentialing qualification and a Non-Medical Prescribing qualification both with extensive paediatric practice. They will be able to work within the four pillars of practice. Within your professional boundaries you will assess, diagnose and prescribe where appropriate. In addition, you will provide advice, care and management for urgent and/ or primary care patients either via telephone/ video or face to face. This may involve the management of long-term conditions and diagnosed or undiagnosed undifferentiated illness. Your role will include supporting the delivery of policy, procedures as required. The clinician will act as a positive role model supporting junior staff whist working flexibly between urgent care and primary care, where appropriate, to provide safe, clinical and expert care. The clinician working in this role will demonstrate clinical reasoning, critical thinking and will be key to supporting clinical work across the wider healthcare team to meet the needs of patients. They will participate in change for the benefit of patients and/or the organisation and support paediatric care and services where required. The extensive footprint of the organisation can facilitate a broad depth of skills and experience to be developed in numerous environments and with the benefits of exposure to in and out of hours working. The post holder will be required to have the ability to be flexible regarding travel and hours to suit the service/organisational demands and to meet the demands of the changing healthcare environment and the expansion of our paediatric services. The organisation provides paediatric care within all UCCs and primary care services and also paediatric specific services such as: - acute paediatric clinics and Alderhey Childrens Hospital Urgent care streams. You will embrace our values and ensure that they are demonstrated whilst carrying out all responsibilities of the role; we: put patients first; look after our people; give great quality care; lead the way in transforming patient care; contribute towards the wellbeing of our local communities.Key duties and responsibilities include: Clinical Practice Assess and holistically clinically examine patients with both minor and complex needs through history taking, physical examination and appropriate use of investigations to confirm diagnosis. Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Refer and discharge safely ensuring that safety-netting advice is offered. Triage telephone referrals, calls from external clinicians, patients and others into a range of services appropriately as required. Demonstrate a visible clinical profile, providing expert advice and demonstrating high levels of clinical competence in the delivery of patient care as directed by the Clinical Service Lead.Communication Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment. Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating. Anticipate barriers to communication and take action to improve communication. Maintain effective communication within the organisation and with external stakeholders. Ensure awareness of sources of support and guidance and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriateNon-Medical Prescribing If you are employed in a prescribing capacity, you must only prescribe within your scope of practice and comply with the gtd healthcare NMP governance registration processes. You must maintain your knowledge and skills and prescribe within your clinical competencies. The non-medical prescriber is required to comply with organisational NMP governance processes and adhere to local and national prescribing guidance. Delivering a Quality Service Recognise and work within own competence and professional code of conduct as regulated by the NMC/HPC etc. Understand own role and scope and identify how this may develop over time. Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures. Prioritise, organise and manage own workload in a manner that maintains and promotes quality. Ensure professional revalidation requirements are met. Deliver care according to NSF, NICE guidelines and evidence-based care. Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation. Initiate and participate in the maintenance of quality governance systems and processes across the organisation and its activities. In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate. Support and participate in shared learning across the organisation and wider organisation. Understand and apply guidance that supports the identification of vulnerable and abused children and adults, and be aware of statutory child/vulnerable patient health procedures and local guidance. Leadership Personal and People Development Lead on shift, ensuring flow and safety of patients and support other members of staff as required. Take responsibility for own learning and performance including participating in clinical supervision and acting as a positive role mode. Act as mentor and resource person for all trainee Advanced Clinical Practitioners or ACPs within your clinical area as appropriate. Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice. Encourage others to make realistic self-assessment of their application of knowledge and skills, challenging any complacency or actions that are not in the interest of the public and/or users of services. Critically evaluate and review innovations and developments that are relevant areas of work. Contribute to the development of local guidelines, protocols and standards. Maintain effective communication with those responsible for the overall commissioning and procurement process. Managing Information Use technology and appropriate software as an aid to management in planning, implementation and monitoring of care, presenting and communicating information. Review and process data in order to ensure easy and accurate information retrieval for monitoring and audit processes.Learning and Development Undertake mentorship where appropriate. Disseminate learning and information gained to other team members in order to share good practice and inform others about current and future developments (e.g. courses and conferences). Assess own learning needs and undertake learning as appropriate. Provide an educational role to patients, carers, families and colleagues in an environment that facilitates learning.
