The Role: We are currently looking to recruit an experienced Care Assessor to join our team based in Newmarket. The ideal candidate will have extensive experience in care coordination for service users living within their own homes and ideally, a social care background.
We Would Like to Hear from You if You:
- Have extensive experience in care coordination for service users in their own homes
- Have a background in social care (preferred)
- Possess a good sense of humour
- Are professional and hardworking
- Have a caring and kind heart, with a willingness to make a difference
- Are a quick learner
- Have excellent organizational skills
- Can grow and expand care rounds, looking for opportunities of growth
Your Responsibilities Will Include:
- Providing support to homecare assistants
- Maintaining positive relationships with service users and staff
- Conducting risk assessments
- Reviewing service user care plans
- Creating and updating care plans

Requirements:
- Driving license (essential)
- Ability to hold the on-call phone on a weekly rota basis
- Willingness to attend care calls as needed
