JOB DESCRIPTION

Post Title: Primary Care Network (PCN) Service Coordinator

Hours: Up to 37.5hrs

Salary: Up to £23,900 PA (Pro Rata)

Line Manager: PCN Operational Lead

Base: Westcroft House Surgery

Job Summary

As an Administrator working for the Primary Care network you will work alongside a variety of patients to ensure that patient care is delivered to the highest possible standard in a safe and efficient way.

As an Administrator working under the general direction of the Senior PCN Coordinator you will undertake a range of administrative duties including maintaining administrative processes and records to enable the team to deliver effective community services. You will ensure that the office environment is safe, appropriately maintained and equipped all staff and visitors.

The role will include coordinating visit appointments via EMIS (clinical database), liaison with various primary care staff, practices and patients, maintaining a visiting schedule, coordinating calls and collating referrals for a variety of services.

The hours of work are designed to provide the required administrative support to the PCN hours; therefore you will be expected to work in line with general practice hours between the hours of 8.30am-6.30pm Monday to Friday.

You will be required to be flexible and able to provide cover for other staff which may involve working at different locations within the locality.

KEY RESPONSIBILITIES

Carry out activities to support clinical triage of referrals for all PCN services.

Schedule visits to patients on behalf of Clinicians and Assistants using electronic systems.

To answer telephone calls as appropriate of the protocols of the service.

Input and retrieve data using IT systems, ensuring that all information taken is recorded quickly and accurately.

To communicate effectively and consistently both with colleagues and callers to the service whilst retaining the ability to remain calm in difficult situations.

To prioritise calls requiring immediate attention of a clinician.

Respond appropriately to enquiries from other services and patients, referring to other members of the team when necessary.

Ensure adequate clinical and office supply stocks available for teams

Provide general and administrative support to team.

Prepare sets of patient notes for use by the teams in patient's homes.

Collect patient and staff feedback, comments and complaints.

Carry out health and safety tasks including the role of Fire Warden (dependent upon location)

Carry out occasional reception duties for the Practice/Hospital/Clinic/office (dependent upon location).

The post holder will work under the direction of the PCN Operational Lead and Senior Network Coordinator.

You may on occasions be required to use your judgement e.g.

You will be expected to anticipate problems and be able to resolve them without being asked

Work with minimum supervision

There are times when you may work alone and you will need to make decisions yourself taking appropriate action.

The post holder will receive regular management supervision

The post holder must maintain the confidentiality of information about patients' staff and Trust business in accordance with the General Data Protection Regulations (GDPR), Data Protection Act 2018 and Caldicott principles.

Employees must be aware of the responsibilities placed upon them under the Health & Safety at work Act (1974), to ensure that the agreed safety procedures are carried out to maintain a safe environment for employees and visitors.

All employees have a duty for safeguarding and promoting the welfare of children and adults at risk. Staff must be familiar with the Safeguarding Policy and the process for raising concerns about the welfare of anyone with whom they have contact. Staff must also ensure they receive the appropriate level of safeguarding children and adult training.

COMMUNICATION

To liaise and interact with multi-professional teams across organisational boundaries, including primary care, social services and secondary care providers according to patient needs and appropriate care pathways.

To maintain communication with all healthcare professionals and outside agencies to ensure seamless, patient-centred service provision.

To ensure effective communication so that the patient receives the appropriate level of care and wherever possible avoid unnecessary hospital admission.

To take an active role in maintaining expert communication with all patients, carers and relatives which meets their individual needs.

Communicate confidential and sensitive information to patients, relatives and carers in relation to their condition

To deal with complaints in a calm and courteous manner, ensuring that, wherever possible, complaints are dealt with efficiently and satisfactorily on a local level.

To maintain contemporaneous accurate and systematic documentation of patient consultations, consistent with legislation, policies and procedures.

This is not meant to be an exhaustive list of duties. The need for flexibility is required and the post holder is expected to carry out any other related duties that are within the employee's skills and abilities whenever reasonably instructed.

Qualifications

GCSE Maths and English or equivalent NVQ 2 in Business Admin or equivalent relevant experience

Experience

Able to demonstrate previous office experience Able to demonstrate experience of regular working with electronic management information systems, Able to demonstrate ability to work without direct supervision Able to demonstrate experience of telephone and electronic communication within own job and with external organisations

Knowledge

Able to demonstrate understanding of office procedures and relevant statutory regulations Able to demonstrate working knowledge of Microsoft Outlook and Microsoft Office Able to demonstrate understanding of issues related to confidentiality. General understanding of relevant legislation e.g. Data Protection, Health and Safety
