Sefton Community Diabetes Team have an exciting opportunity for a Diabetes Educator to join their team. The community diabetes team offers a wide range of services to patients across the Sefton footprint and Diabetes Education is part of that service. Education sessions are held at various sites across the borough of Sefton and the successful post holder will have the autonomy to further develop the education offer we have for patients and also engage with other disciplines in providing Diabetes Education. The successful post holder will work as part of the Community Diabetes Team they will be an integral part of the team and will work closely with the Diabetes Nurse Specialist, Dietetic Colleagues, GP'S and District Nursing Colleagues as well as the wider MDT. The Diabetes Educator is responsible for the co-ordination and delivery of the Diabetes structured education programmes for people with Type 1 & 2 Diabetes across Sefton. This includes the monitoring and development of pathways, systems and processes which are involved in the delivery of these services to ensure these are delivered to a high quality. The post-holder will liaise and work with all participants in the provision of Type 1 and Type 2 diabetes education programmes including, Diabetes Specialist services in Sefton, primary care providers, commissioners and voluntary organisations. The Diabetes Educator will act as the principle point of contact for the programme and also be responsible for the collection, monitoring, collating and reporting of service outcomes required by the different commissioning bodies, ensuring they are produced in a timely fashion Mersey Care is one of the largest trusts providing physical health and mental health services in the North West, serving more than 1.4 million people across our region and are also commissioned for services that cover the North West, North Wales and the Midlands. To ensure the delivery of accessible and effective structured education for people with Type 1 & 2 Diabetes programmes for the residents of Sefton. 2. Monitor and assess the functioning of the programme, identifying and solving problems as they arise working with Admin team, Team leader and other relevant staff within Mersey Care, Primary Care and other relevant bodies. 3. Be responsible for the identification, examination and booking of suitable venues to deliver the educations sessions on a rolling timetable. 4. Plan and organise for the continuous delivery of the programme ensuring sessions are available in all areas as per contractual agreements and taking into account fluctuations in patient demand. 5. Ensure appropriate and robust communications channels exist with the diabetes team for the structured education programmes, the admin team, senior leadership team and all other relevant individuals and organisations. 6. Be responsible for evaluating service standards against the local KPIs and work closely with the Professional lead team to develop the service where appropriate and in response to changes in NICE guidelines. 7. Be responsible working with Business Intelligence for the collection, monitoring, collating and reporting of service KPI and activity outcomes required ensuring they are produced in a timely fashion against the reporting timetable. 8. Assess and evaluate the ensure it is designed to facilitate patient attendance and work with primary care colleagues to promote patient education as part of routine diabetes care. Leadership Requirements 9. Contribute to the development of policy and services to meet the needs of Sefton Primary care provider service defined group clients. 10. Provide leadership, mentorship and role modelling to support staff within the Diabetes team 11. Attend mandatory training, core training and training that will enhance practice. 12. Provide support and knowledge to support staff as required. 13. Act as an advocate and champion for clients. 14. Work within guidelines to identify and manage risk, reporting identified risks to the Team leader and complete RADAR as required. 15. Work in collaboration with other agencies, according to Trust policy. 16. Identify own needs and prioritise work appropriately. 17. Maintain confidential and comprehensive patient records ensuring Mersey Care's Record Keeping, data transfer and data protection policies are followed. 18. Liaise regularly with all stakeholders in the promotion of the diabetes service. 19. Support the diabetes service in the implementation of practice development, improvement and service modernisation initiatives. 20. Develop skills and competencies as required, to provide effective care and support to client group. 21. Monitor all the education resources, ensuring adequate supply and all the teaching aids are in good order, arranging for replacements when needed. Research and Audit 22. Provide relevant data and information as requested in a timely manner. 23. Participate in audits as required by Mersey Care. Clinical Governance / Quality 24. Participate in individual & group clinical supervision. 25. Support the training and development of support staff. 26. Ensure education is delivered incorporates Service Spec and NICE guidance. 27. Regularly review the education sessions to ensure they meet the needs of those from hard to reach backgrounds i.e., language / disability / ethnicity / religion / cognitive ability To work with other professional and statutory groups involved in care management to: 28. Communicate sensitive and complex information, concerning safeguarding adults. 29. Communicate sensitive and complex information to clients, carers, colleagues and other professionals where there may be barriers to understanding, utilising interpreting services and aids to communication where appropriate. 30. Establish robust communication channels both with the Diabetes team, the wider division and other professionals and agencies. 31. Attend staff meetings, team meetings, working groups and focus groups that are relevant to you 32.
