Job Details:

Shifts: 15:45 - 22:30 (Bank positions also available)

Responsibilities:

  • Personal care assistance
  • Medication prompting and administration.
  • Building meaningful relationships with clients.
  • Meal and drink preparation.
  • General household tasks, including laundry, shopping, and housework.
  • Support in accessing the community.
  • Assisting clients in achieving their goals and aspirations

To succeed as a Community Care Practitioner, you will:

  • Be respectful, empathetic, creative, and caring.
  • Possess excellent communication skills.
  • Demonstrate reasonable written and verbal communication abilities.
  • Be willing to undergo a Disclosure and Barring Service (DBS) check (paid for by HCC).
  • Hold a Full UK Driving License and have access to your own vehicle.
  • Be available to work ALTERNATE weekends.
