Job Responsibility:

The post holder will support the operational delivery of clinical services by taking an active role in supporting STT and MDT teams with the support and management of care pathways for cancer patients. The post holder will be the key person identified for providing information and support to the MDT with key stages of the patient's pathway, to enable people to navigate the health and social care system. The aim of the role is to make sure that people living with cancer can access the right support, at the right time. Responsible for joining up services, coordination, providing information and improving communication between care givers, treatment providers and their families, to create a seamless patient experience. The post holder will co-ordinate care by providing a single point of access for cancer patients including rapid re-entry into the system for those people identified as having urgent or specialist needs. The purpose of this role is to complement the existing STT Teams including the multidisciplinary team (MDT) and support patients through the diagnostic pathway and treatment pathways, if a cancer is confirmed and follow up. The scope of the role will vary between Lower GI, Upper GI and Hepatobiliary depending on the STT priorities as determined during the set-up and development of the role. The role will have core elements that are applicable to all roles regardless of their setting and additional elements can be added according to local need. The successful candidate will be responsible and accountable for their practice and behaviour under the guidance and supervision of Management and Leadership team. If a patient is on a 2WR pathway and diagnosed with cancer, the navigator will be a point of contact for the following: To liaise and alert the clinical tea​m if required To chase scan appointments, coordinate results, appointments and queries related to these aspects Signposting to support services As part of the wider clinical team and MDT to support patients and their families with the elements of a person centred approach to their care. Organisational Act as a role model by maintaining credibility across the Trust, the wider health care community and external agencies, ensuring a positive image of the Trust is maintained. Ensure compliance with Trust policies, procedures and guidelines, taking action / alerting the Clinical Nurse Specialist if practice appears to contravene policy, or if concerned about any aspect of patient care. Participate in meetings internally or externally to the Trust, relevant to specialist area of practice, proactively contributing to the meeting by providing advice and expertise in own specialist area. Support the team in the effective use of resources, e.g. budget, supplies and material. Maintain systems and processes to promote a healthy, safe and secure working environment, ensuring compliance with legal and regulatory requirements, maintaining accurate documentation and reporting any concerns. Contribute to the development and maintenance of relationships with other departments, wards within the Trust and external agencies, charities, and regional cancer centres. Work collaboratively with clinical / non-clinical services internal and external to the organisation to provide a seamless service which is responsive to individual patient's needs. Liaise with all members of the extended MDT to aid facilitation of a collaborative approach to the care of the patient. Be flexible in approach, able to exercise initiative and demonstrate a consistently high standard of professionalism, being aware of the need for confidentiality and integrity. Support and contribute to audit processes, governance, research, clinical research trials and service development. Education Demonstrate willingness to identify own learning and development needs and willing to undertake further training. Take responsibility for own learning and development by recognising and taking advantage of all opportunities to learn, including full participation in appraisal, supervision, action learning Administrative Provide a wide range of administrative and clerical support to the clinical team including data collection, stock taking, report writing, including weekly sitreps Support the MDT process to ensure compliance with cancer performance standards and reduce delays in the diagnostic pathway Support the information needs of the individual cancer teams e.g. Quality Surveillance programme Assist the team to complete audits and respond to the findings to improve service delivery Create and maintain databases which record all support and information activity and information resources for audit Maintain accurate, comprehensive and up to date documentation in line with the policies of Bedfordshire Hospitals NHS Foundation Trust Ensure compliance with policies, procedures and clinical guidelines for self and others Co-ordinate the handover with other teams to facilitate safe and effective transition of care between services in order to provide seamless support for people The post holder will demonstrate excellent organisation skills. Type letters where required to expedite patients efficiently through the pathway Identify themes in tracking delays, vs themes in clinician admin or delays in reporting and provide sitreps for the senior leadership team to ensure strategic steps can be taken to mitigate these. Make and change OPD appointments where needed to expedite next steps Confidently report findings and work closely with MDT and STT co-ordinators Set up initiative for efficient and timely clinician admin. Detailed responsibilities The lower, upper GI and Hepatobiliary STT service have developed a pathway for the 2WR referrals for suspected colorectal cancer to meet the 28 day to diagnosis pathway. The post holder will be joining a dynamic, well organised and efficient team committed to delivering high standards of care to this group of patients. To prioritise and efficiently organise own workload on a daily basis to ensure all tasks are completed in a timely manner. To independently prioritise and manage own workload to ensure an efficient service is provided, being flexible and adaptable to change To ensure that all appointments are booked in a timely manner to keep within the pathways targets To prospectively track patients through their 28 and 62 day cancer pathways, utilising the cancer PTL spreadsheet and Somerset system. To liaise closely with all the Nurses, Consultants and all users of the service to ensure that good communication is in place. To monitor Trust systems and ensure that test results are acted upon in a timely manner. Planning and booking virtual clinics to ensure results are acted upon in a timely way, and to monitor the 28day to diagnosis pathway Audio typing of letters to patients as dictated by the Straight to Test team within the expected turnaround time for 2ww pathways and delivering of the 28 day diagnosis. To be responsible for typing patient and professional correspondence from the Cons 2ww clinics / STT assessment clinics and results of diagnostic tests. To liaise and negotiate with the patients, (by phone or in person) booking suitable appointments within the allocated timescale. Making sure all relevant information is sent out within specified timescales. Ensuring all imminent outpatient appointment offers have a valid outcome i.e. patient accepted/declined. This will involve telephone contact with patients. To manage appointment dates. Implement choice at National standards via the choice programme. To be responsible for ensuring that if patients cancel their appointments, the vacant slot is efficiently being offered to another patient. To liaise with GP practices and patients as necessary to obtain all relevant, accurate data if differs from Hospital IPM system To audit referral data from GP practises and report compliance as part of the primary care 2ww referral criteria To contact patients to chase sample return if the results are outstanding Monitor all surveillance making sure they are validated and receive appropriate appointment date To ensure that office protocol and procedures are maintained and reviewed appropriately. Adhering to trust policies and procedures To resolve straight forward and complex enquiries from patients. To give advice, reassurance to patient, carers and relatives who have concerns regarding appointment or procedure. To take incoming telephone calls ensuring they are dealt with in a courteous and sensitive manner with due regard for confidentiality. To be the first point of contact for staff using own initiative and decision making skills to deal with complex situations. To communicate effectively with medical professionals at all levels and the general public using experience and judgement To use experience and judgement to escalate any relevant information or issues to the Service Manager. To be proficient in the use of spreadsheets and databases. Keeping information updated. Be part of a team to carry out continuous service audits and ad hoc audits and surveys
