Job Responsibility:
Generic requirements of the role/s : This role requires the individual/s to work with General Practitioners, Clinical Leads, ICB Management Teams, and a broad range of other professional colleagues and organisations: Work collaboratively at PCN level, with an increasingly diverse group of professionals, public partners and third sector organisations to develop an integrated and collaborative working environment Demonstrate excellent organisational skills to develop appropriate networks, interfaces and liaison with partner colleagues including LMC and GP Alliance, as well as ICB staff to support the development of PCN working Demonstrate and exemplify positive behaviours and attitudes which will support co-operative and partnership working Respond proactively to emerging issues and risks identified within the PCN, drawing on expertise of local practices, appropriate ICB support staff and partners to find local solutions where possible and facilitate early intervention and mobilisation of support as required Promote a culture of continuous quality improvement and patient centred care, giving co-ordination support to the PCN to maintain high professional standards. Be aware of perceived or actual conflicts of interest and take appropriate steps to minimise any unintended negative impact, challenge or reputational damage which could arise. Co-ordination and Project Management Requirements: Work with the PCN Clinical Leads to establish the PCN network across designated practices. Provide project management and Organisational skills to a designated PCN of general practices. Support collaboration and engagement between the PCN practices and future place based teams to develop effective care network across the PCN. Support the Clinical Leads to actively engage with all practices within the PCN. Set up communication channels to facilitate collaboration and joint working, including coordinating PCN level meetings. Co-ordinate the development and production of a shared PCN action plans; Undertake a range of project management actions to ensure that agreed action plans are achieved within agreed time scales; Provide timely reports and updates on PCN issues and progress against the agreed PCN plan; Work with practices within the PCN to identify resilience, business continuity and business process improvements and implement resilience plans across practices within the PCN and across PCNs; Ensure excellent communication between all practices and staff working within the PCN; and collaboration as appropriate with other PCNs. Work collaboratively with other partners and their staff teams deployed to deliver services to the PCN; Ensure appropriate relationships and connections are developed and maintained; Attend key meetings and provide full secretariat support as required to discharge co-ordination function; Provide PCN progress updates as requested by the ICB Professional Conduct and standards of behaviour: Engage personal and professional expertise to work collaboratively with the practices in their assigned PCN, forming close working relationships with a range of internal and external professionals, exploring opportunities for collaboration, resource sharing, shared service delivery and the ongoing development of services. Work closely with PCN clinical lead colleagues, the Primary Care Development Team and broader Management Team, engaging on operational and strategic matters ensuring practices within the PCN respond coherently to existing and future demands. A high degree of organisational ability and awareness, coupled with strong engagement skills and the ability to influence and mobilise colleagues to achieve agreed tasks and actions is expected. Public and Community Involvement Maintain the profile of the Public Involvement agenda in all activities through the sharing of information & current best practice guidance. Working with the ICB engagement and localities team engage with local community representatives and public groups regarding ongoing redesign initiatives and service planning. Ensure practices within the PCN maximise utilisation of community assets and social prescribing through active sign positing and care navigation.
