Job Responsibility:
The Individual Funding Team provides a county-wide service, and is essentially responsible for managing mental health funding processes, when an external package of care is required. The team's profile has grown in recent years, with an increasingly diverse role, and it has become an integral and embedded resource within the organisation. The team works closely with the Local Authority when joint funding is required. The post holder will hold a Nursing, AHP or Social work qualification and will be working in an integrated skill mix team, including a Service Manager; Operations Manager; qualified Nurse and AHP clinicians; a Business Manager, and other administrative staff. The post involves brokerage and review processes, to evaluate the need for, and to source and review care packages for eligible adults. This requires close liaison with other community mental health and learning disability services; in-patient services; the Local Authority, and a wide range of non-statutory service providers. The post involves frequent travel, between the organisation's bases, and to local and more distant locations, where care packages are being delivered. NHFT is an integrated primary care and mental health Trust, providing physical, mental health and specialty services in both hospital settings and out in the community. We also provide health services to various prisons and detention centres in Bedfordshire and Cambridgeshire. PRINCIPAL DUTIES The role of the Clinical Assessor is divided into two main functions, brokerage of an Individual Package of Care (IPC), and/or reviewing of an IPC.
Clinical Assessor function: Participate in weekly referral meetings to discuss eligibility of referrals Attend professional, clinical, MDT meetings and CPA reviews Ensure all statutory service options have been explored Identify and review service user care package needs using clinician-rated assessment Identify the appropriate and least restrictive IPC for service users Quality check placements against set standards (CQC or local) Quality check the assessment, care and support plan and monitoring of the service user prior to commencing an IPC Prepare the documentation for the funding panel for an IPC/placement Organise and conduct a review of the IPC and complete relevant review documentation
- Identify a new IPC/placement for a service user when step down/up is clinically appropriate
Inform the Business manager of any changes to the IPC Identify an early placement for service users in acute settings who may require an IPC Identify if the service user is eligible for an alternative funding stream or package
