Job Purpose
Boston PCN has a Wellbeing Team to support the holistic needs of identified cohorts of patients. The team is multidisciplinary and will work together to support the varied needs of this patient group as well as linking in with other local services and the third sector. In addition, we are growing our Home Visiting Service, which provides advanced nurse practitioner visits to people who have a same day need and can't get to their GP practice. We also work closely with our Neighbourhood Team.
These community-based teams work to provide care out in the community. They require an enthusiastic and organised coordinator to provide administrative support.
The Role
The team coordinator will provide administrative and coordination support to the multidisciplinary teams and patients.
Key outcomes of the teams are:
- Holistic support for patients to better manage their condition
- Supporting patients who will benefit from proactive health management
- Enabling people to remain in, or close to, their own home, whenever possible
- Improving pathways between services, reducing inefficiencies, fragmentation, and duplication of care
- Promote and encourage access to community based public health initiatives such as healthy eating, physical activity and social connectedness
- Individuals being better equipped to self care, relying less on GP and urgent care services
- Supporting complex patients through a multidisciplinary based approach
- Providing urgent on the day visits on behalf of GP practices
The Team Coordinator will provide a crucial interface between the multidisciplinary teams, GP practices and patients and will have overall responsibility for the coordination of meetings, planning and tracking patient caseloads as well as recording data to assess outcomes. In addition the coordinator will support lone working and effective use of resources.
Responsibilities of the Post Holder Responsibilities for direct/indirect patient care
- Assist patients during incidental contact e.g., signposting and answering routine enquires.
- There will be occasional contact with service users, carers, and the public, while providing information regarding services
Responsibilities for policy and service development implementation
- Follow policies/implement policies and propose changes in working practices or procedures for own area e.g., comments on proposals / implements policies by devising and maintaining efficient systems, sourcing referral criteria, and ensuring this is accessible to all relevant parties.
Responsibilities for financial and physical resources
- Personal duty of care in relation to equipment / safe use of equipment e.g., use of office equipment.
Responsibilities for human resources
- Demonstrate own activities to new or less experienced employees
Responsibilities for information resources (including systems access)
- Regular requirement to develop or create reports or documents. Responsible for maintaining one or more information system, which is a significant job responsibility e.g., maintains databases to collect and monitor information, maintains and updates to ensure patient details are correct and track patient care management.

Qualifications, knowledge, training, and experience required
The successful applicant will have:
· Minimum GSCE Grade C or equivalent in maths and English
· Experience in using Microsoft Excel or similar database systems
· Strong administration skills
· A clear understanding of service user confidentiality
· An understanding of health and social care processes
· Advanced IT Skills - Microsoft Office
· Excellent communication skills both written and verbal
· The ability to work as a team
· The ability to prioritise and manage own workload
· Analytical skills
· The ability to interpret information and present it in a clear and concise manner
· The willing to work flexibly and provide cover as required
