Job Responsibility:

To provide strong and effective clinical leadership and line management for Manchester teams promoting a positive and therapeutic culture that empowers staff to achieve optimal performance. Pro actively utilises own judgement to provide options and resolutions which may involve complex facts. Optomising all professional advice from the MDT, family, carers to drive a way forward for example: case conferences, vulnerable adult panels, guardianship applications Oversee the daily delivery of discharges including complex cases and patients clinically ready for discharge. To provide effective leadership for the Manchester Patient Flow Service spoke practitioners, promoting a positive and therapeutic culture. Support the Care group at internal and external patient flow forums, sharing information, supporting improvement plans and taking issues forward. Participate in local and regional networks to promote Manchester care group and patient flow Retain daily oversight of discharge progress with live and accurate updates to the Head of operations, Associate directors and Strategic lead as required.
