Job Title: Occupational Therapist / Physiotherapist

Band: 6/7

Trust:Central Surrey Health

Location: Woking Community Hospital, GU22 7HS

Ward: Urgent Community Response

Pay Rates: £27.09 - £31.60 Per hour

Shift Patterns: Full/Part time across 7 days / Mix of shift patterns between 8am-8pm

Why might this job interest you?

Youwill work as a member of the multi-disciplinary Urgent Community Response (UCR) team. Undertaking assessments, clinical diagnosis and treatment of patients including telephone triage, multi-factorial falls assessments, and patients who may require crisis management and/or rehabilitation interventions to prevent acute hospital admissions and to support complex hospital discharges. These patients often have complex presentations.

What you can add:

· Promotion of the service to others e.g. to visitors.

· To contribute to and participate in team objectives and service developments.

· To make diagnosis of perceptual/cognitive and functional difficulties from generic or specialist assessment.

What you'll need:

· Degree or Equivalent in Physiotherapy / Occupational therapy

· HCPC Registration

· Minium of 2 years post qualification & NHS Experience

· Knowledge of a wide range of client groups including any of the following: community, rehabilitation, hospital, neurology, elderly, oncology and orthopaedics

In addition to experience, all Qualified roles will be required to provide evidence of

  • Relevant Qualifications
  • Registration with the relevant Registration Body if applicable
