Job Responsibility:

This post is to support the call handling in a busy Intermediate Care Team. We are actively seeking a dynamic and adept call handler to join our dynamic team during out-of-hours (OOH) periods. The individual selected for this role will additionally contribute to service enhancement initiatives and engage in comprehensive data collection efforts. The role involves working in a mixed team alongside administrative staff and close collaboration with the Integrated Care Team. It is a crucial, customer-oriented position, serving as a liaison with patients, families, healthcare professionals, and external stakeholders referring to NHFT Community Nursing Services. Utilizing System One and referral pathways, you will record patient demographics and basic clinical information, directing patients to the most suitable healthcare professional or final disposition. Referral methods encompass telephony, email, and electronic health systems. The role requires collaborative efforts with service colleagues to ensure an outstanding level of patient service.
  • Excellent interpersonal skills and the ability to utilise them to communicate effectively within a multi-disciplinary and team based working environment.
  • Use effective communication skills to holistically assess patient's needs and determine relevant cultural, social, economic factors in a respectful and non-judgmental manner.
  • Provide a professional, courteous and efficient telephone service consistent with organisational standards, managing calls in a professional and reassuring manner at all times.
  • Participate in audit, customer satisfaction programs, research and other high quality improvement activities.
  • Process referrals, verifying data is correct and entering on to SystmOne. Be able to recognising the urgency of information sharing and prioritise allocation/referral.
  • Be responsible for the organisation of sessions/clinics/administrative tasks (updating ledgers on SystmOne (adding removing as appropriate) and bringing to Team Leaders attention when appropriate) which meet the planned needs of the team
  • Ability to maintain contemporaneous records using computer software in line with local / national policies, guidelines and protocols.
  • To ensure correct referral onto appropriate community service including community nursing, 999 ambulance and GP.
  • Identifying issues which may negatively impact upon service delivery and reporting these onto the shift supervisor in a timely manner.
