Job Responsibility:

Job Title: Patient Pathway Coordinator

Location: Alder Hey Hospital, Eaton Road, Liverpool L12 2AP

Hours: 37.5 per week Monday - Friday 9.00am - 5.00pm

Trust Location: Alder Hey Children's NHS Foundation Trust

What you'll be responsible for:

· The purpose of this post is to provide a comprehensive administrative service within the Clinical Business Unit. The post holder's main duties will include planning, organising, directing and coordinating the work of the department. The Pathway Co-Coordinator will be expected to work with the minimum of supervision

· Communicate professionally and effectively with patients/parents/carers, the general public and all internal and external agencies both verbally and non-verbally in a timely manner,

· Undertake a key role in ensuring the delivery of a high quality patient pathway by working closely with clinical teams to meet national and locally agreed targets for both in-patient and out-patient episodes by monitoring clinic templates, waiting lists and working closely with the management team to address any issues

· Co-ordinate investigations and follow up on results, ensuring all results have been brought to the attention of the clinical team. Establish a clear audit trail to ensure actions have been dealt with.

· Minute Taking duties

You'll learn the following whilst working at the trust:

1. An in depth understanding of the roles and responsibilities involved in working within the NHS

2. Knowledge of the systems used, to effectively complete your role to the highest standard at all times

3. A sense of teamwork, gained through working alongside and supporting colleagues from all levels, within the organisation

You'll have the following skills/experience:

· Alder Care / Meditech system knowledge

· Previous NHS experience

· Minute taking experience

· Proficient with all Microsoft Office packages

· Excellent communication skills, both verbally and non-verbally when dealing with patients/relatives and other internal and external agencies especially when providing or receiving complex or sensitive information
