Job Responsibility:
An exciting opportunity has arisen for 2 motivated and enthusiastic people to join Area 1&2 Secondary Care Team as a Community Support Worker. The Team provides monitoring and support to older people with enduring and complex mental health needs. The post holder will be required to work as part of a team providing support to patients in receipt of Secondary Care Mental Health Services as defined by Mental Health Measure 2010. Assisting individuals to achieve and maintain optimum levels of independence by providing support, choice and dignity in their daily living via assessment, implementation and evaluation of agreed plans of care. The post holder is required to work alongside the Registered Nurse in providing and monitoring an enhanced level of support ,assessment and treatment to patients under Secondary mental health Care in the community. Working collaboratively with the Multi-disciplinary team (MDT) to ensure the maximum benefits to patients/residents using a patient centred approach to care. Observing and monitoring medication side effects, physical observations and mental health of patients and reporting these to the MDT. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
