Job Responsibility:

An exciting opportunity has arisen to join Brent Community Rehab's Discharge to Assess Team. We are looking for administrators, rehab assistants and physiotherapists to work closely with Brent Adult Social Care colleagues working with Reablement to deliver world class reablement to Brent residents. These roles are fixed term contract roles until 31 March 2025, with the possibility to extend beyond this date. This is a great opportunity to experience truly integrated working in a supportive environment to provide high quality patient centred care in the community. Band 7 physiotherapy applicants should have experience working at a senior level in a wide range of clinical areas. You will be provided learning opportunities and support to develop your skills e including clinical supervision, appraisals and training.   • Providing highly specialist physiotherapy care to a community based caseload.
  • Carrying out robust triage of patients referred to the team to ensure their safety and optimal pathway allocation
  • Providing clinical leadership and support to the AHP team to provide education and supervision of junior therapy staff, rehabilitation support workers and students.
  • Supporting and deputising for the operational manager of the service
