We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.
The shift pattern for this post is Monday - Friday 8.30am -4.30pm .
The Community Respiratory Team is a dynamic and responsive multidisciplinary team (MDT) that aims to support patients with moderate to severe COPD within Glasgow City in their own homes. The team is comprised of Physiotherapists, Nurses, OT, Pharmacist, Dietitian, Rehabilitation Support Worker, Business Support and Leadership roles.
It is an evidence based service with the MDT supporting three distinct intervention pathways:
- Prevention of hospital admission (support for acute exacerbation)
- Early supported discharge
- Chronic disease management (rehabilitation and self-management) The successful candidate will work as part of the team to provide holistic, timely, individualised and clinically effective care, including assessment, treatment to support effective exacerbation management and avoidable hospital admission, maximise function and promote self-management. You will support the early discharge of COPD patients from hospital.
You will link with the Respiratory Early Supported Discharge (ESD) team, and hospital based respiratory assessment services/respiratory nurse specialists. It is essential that you have a Degree in Physiotherapy and should be currently registered with the HCPC. This post requires membership of Disclosure Scotland's PVG.

NHS Scotland is reducing their full time working week from 37.5 to 37 hours per week from 1 April 2024 but with no change in pay. This reduction will also be applied pro rata for part time staff. This advert and any subsequent offer/contract of employment therefore reflects the new working hours. However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period for which you would be paid until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.
